For this project, create a paper that addresses the following:
Identify a health concern, behavior, or disease facing your community or another area. Explore how this health concern, behavior, or disease affects different groups within the population (i.e. age, race/ethnicity, gender, etc.) and discuss these differences.
Next, identify how this health concern, behavior, or disease is related to social determinants of health outlined in Healthy People 2030. Be specific and provide data on how these determinants led to the health concern. Does your identified concern vary by different populations within the community and why?
Discuss how the community plans to provide resources and culturally sensitive and responsive care and public health services for the most affected populations.
Lastly, include any stakeholders or people who should be involved in ensuring this program or project is successful in the community.
